Registration Information

Parents should bring the following information when registering their child:

1. Immunization Record (must include immunizations for 3 HEP B, 2 MMR

  and Tdap (Tdap  must have been given within the last five years)

2. Birth Certificate

3. Proof of Residency

4. Previous report card

5. Proof of guardianship/custody (if applicable)
