Transitions

The Transitions mission is to provide life skills and vocational training to students with intellectual and developmental disabilities, ages 18-21. The Transitions program bridges the gap between the classroom and the real world by fostering positive relationships within the community and empowering students to become valuable members of the workforce.

What is Transitions?

Transitions is a community-based program in downtown Warrenton, specifically designed for post-high school special education students in Fauquier County Public Schools. Located on Main Street, Transitions provides hands-on work experiences and life skills education in a small group setting outside the student’s base school. Through research-based methods, the program develops a Person-Centered Plan (PCP) for each student, helping them cultivate skills toward greater autonomy. Our goal is for students to leave FCPS ready to live fulfilling lives as integral members of their community.

What Transitions Offers:

  • Community-Based Learning: Students engage in real-world, hands-on experiences in Warrenton, learning essential skills outside the traditional classroom.
  • Vocational Training: Job coaches work with local businesses to offer students personalized job training and vocational skill development.
  • Life Skills Development: Instruction focuses on independent living skills like financial literacy, personal care, meal preparation, and household management.
  • Social and Community Involvement: Daily practice of social skills and community engagement fosters relationships and self-confidence.
  • Self-Determination: Students are guided to take control of their own lives, learning self-advocacy, time management, and communication skills.

Who Can Participate?

The program is open to Fauquier County Public School students with intellectual and/or developmental disabilities, ages 18-21, who have earned an Applied Studies diploma. Participation is determined through an application process.
