Important Notice: Please have students come to the clinic when ill instead of calling home first. The school nurse will assess and then make the appropriate phone call.

 Common Health Forms

All rising seniors for 2021-2022 school year will need to have at least one meningococcal vaccine (MenACWY) vaccine that was administered at the age of 16 or older. If your student had their 1st dose earlier than age 16 then they will need a 2nd dose prior to beginning school in August 2021. Documentation must be provided to the school nurse by the first day of school. You may email documentation to loritabrown@fcps1.org, fax (540) 422-7359 or drop it off at school in the main office and it will be placed with school nurse's mail.
