VHSL Physical Form

Beginning July 1, a student’s physical will be valid for 14 months from the date the physician signs the form. This replaces the previous policy, which required physicals to be valid from May 1 through June 30 of the following year. The VHSL Sports Medicine Advisory Committee has revised the form to reflect this change.

To participate in VHSL athletics, pages 1–3 of the updated physical form must be submitted to the school. Please note that the form expires 14 months from the practitioner’s signature date on page 3.
